As a medical spa, we wanted to spread awareness of a sensitive subject that is very relevant in today’s world –Body Dysmorphia. Luckily for us, we have Facial Plastic Surgeon Dr. Chelsea Troiano to give us a little insight on how she handles people with body dysmorphia in her practice.

What is Body Dysmorphia?

It is a mental disorder where there is an obsessive focus on one’s appearance. A perceived flaw can be small or imaginary, but the person can spend hours a day trying to change it. The person may try many cosmetic procedures or exercise excessively. Their negative thoughts can cause serious obsessive-compulsive tendencies and interfere with day-to-day activities. They may avoid social activities including work, school and even isolate themselves from family and friends because they fear others will notice or judge what they see.

Is Body Dysmorphia common?

According to the Anxiety and Depression Association of America, 1 in every 50 people struggle with Body Dysmorphic Disorder (BDD). 

The American Society of Plastic Surgeons reports that up to 10% of patients looking for cosmetic procedures are affected by the [BDD] disorder.

Dr. T tells us that she sees more than these statistics in her practice. At least once per week, she will see someone with body dysmorphia. She adds that it varies in the spectrum, those that need psychiatric help and those who don’t.

Where is Body Dysmorphia developed?

Credible sources online say it is unclear where people form BDD. It varies in experiences, trauma, environments, chemical imbalances of the brain, anxiety, social media, and many other reasons. 

According to the Anxiety and Depression Association of America, you see BDD most often develops in teens and adolescents but can carry into adulthood. It affects men and women almost equally, 2.5% in men and 2.2% in females. 

The American Society of Plastic Surgeons says only 7% of professionals screen patients for body dysmorphia. In your professional opinion, do you think this number is accurate?

“Yeah. It takes time, number one,” Dr. T adds, “And number two, it offends people. They don’t treat this as a medical procedure… I need to know the full medical history because first I am a doctor, I’m there to pick up on any clues if something else is going wrong with you and I can recommend seeing a general practitioner… They have to trust that I am going to do what is in their best interest.”

At her practice, she implements a survey tool to identify the spectrum in each patient.


How do you go about it if someone is very adamant to get a procedure that you know has body dysmorphia?

“I will tell them, ‘Unfortunately, my aesthetic does not align with yours and so, I am not the right surgeon for you.’ Usually, they will get upset but I can’t go to sleep at night knowing I did something wrong to somebody.” – Dr. Traiono

In your professional opinion, would you say body dysmorphia is becoming more prevalent now that social media is a huge part of our lives?

“Yes, social media has increased incidents of body dysmorphia because people see filters and unrealistic expectations of what their face and body should look like, but also because of a great increase in cyberbullying” -Dr. Troiano

According to Facial Plastic Surgeon Dr. Chelsea Troiano, MD
There are certain skin care practices to age gracefully that we hear over and over again from friends, family and our favorite influencers that we attempt to implement into our own routines without thinking twice. We assume they are somewhat credible, but unfortunately that isn’t always the case. Luckily for us, we have Facial Plastic Surgeon Dr. Chelsea Troiano who went through all years of education and practice to give ‘us a little chat about the best ways in medicine to keep our skin looking youthful and radiant.

1. Start anti-aging while you’re young!

Dr. T recommends starting anti-aging treatments during our early 20s because it is easier to prevent aging, rather than to take drastic measures in the future to reverse aging. She encourages young adults to get low doses of neurotoxins (botox) to freeze the facial muscles to avoid forming wrinkles. Dr. Troiano started at 23-years-old. Now, her maintenance is a low dose every 6 months. She gave us a budget friendly tip, “You can try to train your face to not really use those muscles as much so you don’t need to get it every 3-4 months.” Start focusing on areas that first see signs of aging in areas such as between the eyes, forehead, outside of the eyes/ crows feet.

2. Stay out of the sun, use sunscreen religiously even when inside.

You still get UV sun rays through the windows. Even though it’s not direct, the rays still touch your skin. If you can block those rays all the time then you are one step above everyone else. Dr. Troiano emphasized that she uses SPF 50 or higher every single day. It is the best product to slow down the look of aging.

3. No smoking

She says people look decades older when they are smokers. “This includes vaping, for all the youngsters,” Dr. Troiano said. Scientifically, smoking ages us faster because you are depriving your skin of the nutrients. Not to mention the thousands of chemicals that are in cigarettes and vapes. When you combine those with the lack of circulation and oxygen that smoking causes, this leads to the breakdown of collagen and elastin in our skin. These are essential to the strength and hydration of our skin. When all of this is taken away, we, in turn, look older.

4. Staying hydrated

 Your skin is thirsty! This also means low alcohol intake. When we are dehydrated it causes damage to your body, and then your body can’t restore and keep all the cells and skin looking beautiful if it’s constantly repairing all the damage you are doing to it. So, do yourself a favor for your skin and for your overall health and drink lots of water!

5. Cleansing and moisturizing twice a day – minimum

 Sleeping with makeup on is a big no no. You don’t want to trap dirt on your skin as it leads to collagen destruction and in time, premature aging. Regardless of your skin type, it is important to wash your face everyday and remove ant dirt, make-up, oil and grime that has been built up on your skin throughout the day. Our skin is exposed to irritants and pollutants 24/7 — yes, even when we sleep. That is why twice-a-day cleansing is so crucial.

6. Eating a whole foods diet

Feed your skin, mind and soul right. Anything really colorful in fruits and veggies to get all the vitamins and minerals that are good for the skin. Topical products with vitamins are a little helpful but nowhere near as eating them and glowing within. Your skin’s purpose is to protect you from getting things inside, so topical things help a little bit but to get to the root of the problem, usually for skin health it is factored to what we put into our bodies.

7. Not overfilling your face

Dermal fillers can sometimes make you look older based on your age. In your 20-40s, minimal fillers are recommended to keep the natural structure of the face. It’s best to keep fillers minimal until older age when more volume is lost. When you don’t look natural, then people start to notice your age.

8. Sleeping

You need that regenerative time. While we are sleeping, our skin goes through restoration where  it has the ability to retain moisture, protect and heal itself — especially in those deep stages of sleep. If we deprive ourselves of sleep, we are reducing the amount of time that our skin has to repair itself. It’s true what they say, we need our “beauty sleep.”

9. Keep stress levels down.

Stress increases cortozol, a stress hormone which increases damage all over the body including the skin. So take a break, ground yourself because it comes in full circle, the less wrinkles the less we stress and the less we stress the less wrinkles.

10. Consistency is key

All the things mentioned with consistency lead to compounding effects. You can tell a lot about a person through the biggest organ we humans have. Take care of your skin and your skin will take care of you. “That’s the goal, you don’t want anyone to know” that you had botox or filler. She understands that preserving natural beauty is what always looks best. “You just have a whole glow, it’s harmonious and that’s the goal.”

What are the side effects?

Here at Dash Medical Spa we value transparency with our products and services giving you as much information as possible for your discretion. Let’s talk about some issues that may occur with CoolSculpting Elite.

First, let’s talk about the misconception of what many think is a form of treatment for weight loss. This is not a weight loss method, it is toning and contouring of the body by reducing fat cells. When considering CoolSculpting Elite, it will not help with visceral fat which is in the intra abdominal tissue of your stomach that is only reduced with exercise and maintaining a healthy diet. 

  • CoolSculpting does not treat aesthetic considerations such as skin laxity, cellulite, stretch marks.
  • When assessing the face, as fat volume is removed, the appearance of the jaw line may become more apparent.
  • Most patients will experience minimum to mild discomfort and in rare cases moderate to severe.


Is CoolSculpting Elite for Everyone?

No. You may not be considered a good candidate if you suffer from:

  • Cryoglobulinemia: a form of Vasculitis—a family of rare disorders characterized by inflammation of the blood vessels, which can restrict blood flow and damage vital organs and tissues.
  • Paroxysmal Cold Hemoglobinuria or Cold Agglutinin disease caused by premature destruction of healthy red blood cells by autoantibodies.
  • Neuropathic disorders such as post-herpetic neuralgia or diabetic neuropathy.
  • Bleeding disorders or regular use of blood thinners.
  • Skin conditions such as eczema, dermatitis, or rashes in the area to be treated, recent surgery or scar tissue in the area to be treated.

  • It is not recommended for clients who are pregnant, anyone with known sensitivity, allergy to fructose, glycerin, isopropyl alcohol, or propylene glycol.
  • Patients with chronic pain, sensitivity to cold, anxiety disorder may be more prone to pain or discomfort during treatment
  • Do not use CoolSculpting elite

-on areas with subcutaneous fat layer thickness of < 1 cm 

-on areas of decreased sensation or perfusion 

-on areas with minimal underlying muscle mass on areas with superficially coated nerve branches arteries or veins

-on the face, head, genitalia, inguinal creases, axille popliteal fossae, antecubital fossa, hands, or feet


What are the warnings post treatment?

Vasovagal occurs when your body overreacts to certain triggers, such as the sight of blood or extreme emotional distress. Symptoms include dizziness, lightheadedness, nausea, flushing, sweating, fainting.

Late onset pain may occur, on average 3 to 4 days after treatment. The symptoms are self-limiting and resolve completely within approximately 2 weeks. It is self limiting and resolves completely without long term sequelae. The pain can be described as deep pain, stabbing, shooting, burning sensations, hypersensitivity, and severe pins and needles.

First and second degree burn may occur during treatment. It typically resolves with proper care. 

Subcutaneous induration presents as a generalized hardness and or discrete nodules within the treatment area. It may present with pain and/or discomfort.

Hyperpigmentation can occur after treatment and typically resolves spontaneously.

Paradoxical adipose hyperplasia presents as visibly enlarged tissue volume within the treatment area. May develop two to five months after treatment. At this time, there is no evidence of spontaneous resolution of PAH.
